ADMISSIONS PROCESS
ECC Retirement Village's Skilled Nursing program is available to individuals being referred from a hospital or as a member of the community. Individuals who may qualify for admission into the Skilled Nursing program at either our Albright or StoneRidge communities could show signs of difficulty taking their medications, transferring from a bed to chair, dressing and bathing, and shows confusion to where it creates an unsafe living situation. It is important to note that a person does not need to show all of the above symptoms to be submitted into the program.
If interested in entering yourself or a loved-one into our Skilled Nursing program, below is an outline of the admission process:

1. Contact Deb Brewer, Director of Admissions and Social Services. When a hospital contacts Deb directly, they will automatically forward all patient information. Deb can be contacted at 717-866-3504 or via email at dbrewer@eccrv.com or also by fax at 717-866-3291.
2. Complete the application. It is a ten-page form that contains contact information, medical profiling, treatment history, level of functioning, financial information, and hospital history.
3. Sign the release form. If you are applying from the community, you must sign a release form for the Village to receive the potential resident's medical history from the family doctor.
4. Obtain doctor's order papers. A doctor's order is necessary for all admissions from community members.
5. Schedule move-in date. Move-in date depends on the availability of a room.
6. Finish all paperwork. The resident, or Power of Attorney for that resident, must complete the admission agreement, billing consent forms and optional services arrangements.
